Book Initial Psychiatric Evaluation. Sign up for patient portal. Complete consents, credit card authorizations, and questionnaire no later than 48hrs prior to appointment. ($75 Cancellation fee if appointment canceled with 48hrs of appointment)

STEP 2

Day of appointment instructions: Log into session or arrive at office 10 minutes prior to appointment. We discuss your medical history, current symptoms, and formulate a plan of care.

**Virtual visits: Make sure you have a secure Wifi connection, dressed in clothing, not driving, and located in a well lit room.
